- Cast Certificate (if belongs to OBC, SC, ST)
- Original Mark sheet for class 10th for TOC (Transfer of credit)
- Two Latest Colored Photographs (Passport Size)

Eligibility Criteria and Required Documents for NIOS On Demand 12th Admission (Stream 4):
- 12th class Failed Original Mark sheet or Admit Card.
- Aadhar Card is must (Address Proof like – Aadhar card, Electricity Bill, Rent Agreement(if rented)
- Cast Certificate (if belongs to OBC, SC, ST)
- Original Failure Mark sheet of class 12th for TOC (Transfer of credit).
- Two Latest Coloured Photographs (Passport Size).
- Email Id for Confirmation.
